Major Roots Holistics Membership Association Agreement
Private Membership Association Agreement
This Agreement is entered into by and between Major Roots Holistics Membership Association, hereinafter referred to as the “Association,” and the individual applying for, accessing, purchasing, scheduling, or participating in Association offerings, hereinafter referred to as the “Member.”
By submitting a form, checking an acknowledgment box, scheduling, purchasing, signing, or otherwise participating with the Association, the Member confirms that they have read, understood, and voluntarily agree to this Agreement.
1. Private Membership Relationship
The Member understands that Major Roots Holistics Membership Association is a private membership association offering educational wellness support, lifestyle education, member resources, and related private offerings.
Participation is voluntary and based upon private agreement, confidentiality, mutual respect, and personal responsibility.
The Association reserves the right to approve, deny, suspend, or discontinue membership or participation when necessary to protect the purpose, integrity, or safety of the Association.
2. Purpose and Educational Nature
The Association exists to provide private educational wellness information and lifestyle-based support to members.
Educational topics may include food, hydration, minerals, digestion support, lifestyle rhythms, stress support, natural wellness practices, and other root-focused wellness education.
The Member understands that the Association does not diagnose, treat, cure, prevent, or prescribe for any disease, illness, injury, or medical condition.
All information, consultations, resources, forms, programs, and communications are provided for educational and lifestyle purposes only and are not a substitute for medical, legal, financial, mental health, or other licensed professional advice.
3. Member Responsibility
The Member remains fully responsible for their own choices, actions, wellness decisions, and participation.
The Member agrees to use personal judgment and seek appropriate licensed professional guidance when needed, including before making decisions related to health, medications, supplements, diet, legal matters, or finances.
The Member understands that results are not guaranteed and that individual experiences may vary.
4. Confidentiality
The Member agrees that Association communications, educational materials, member resources, forms, survey responses, program materials, and private conversations are confidential and intended only for approved members or intended recipients.
The Member agrees not to copy, publish, distribute, post, forward, sell, or disclose private Association materials or member information without written permission.
This confidentiality provision does not restrict disclosures required by law, disclosures necessary for safety, or disclosures made with proper written permission.
5. Member Information and Communication
The Member understands that information voluntarily submitted to the Association may be used for Association-related purposes, including reviewing membership requests, preparing for consultations, providing educational support, scheduling, communication, and related private offerings.
The Member agrees that information provided is accurate to the best of their knowledge.
If the Member separately agrees to receive text messages, message frequency may vary, message and data rates may apply, and the Member may reply STOP to opt out or HELP for help.
6. Additional Terms
Specific consultations, programs, memberships, payment plans, digital products, or private offerings may include additional terms, disclosures, payment agreements, refund policies, or consent forms.
Those additional terms shall apply to the specific offering in addition to this Agreement.
7. Dispute Resolution
The Member and the Association agree to make a good-faith effort to resolve any concern or dispute privately and respectfully.
If a matter cannot be resolved privately, the parties agree to attempt mediation with a mutually agreed-upon neutral mediator before pursuing further action, unless otherwise required by law.
8. Acknowledgment and Acceptance
By checking an acknowledgment box, submitting a form, signing, scheduling, purchasing, accessing resources, or participating with the Association, the Member confirms that they understand and voluntarily agree to this Agreement.
The Member further confirms that Major Roots Holistics Membership Association provides private, educational, membership-based wellness support and does not provide medical diagnosis, medical treatment, legal advice, emergency care, or guaranteed outcomes.
